AXiS NeuroWellness and Neurological Associates of Long Island offer text (SMS) messages to support secure access to our services, including one-time passcodes for two-factor authentication (2FA) to the AXiS patient portal, as well as account-related and appointment-related notifications. By providing your mobile phone number and opting in, you consent to receive such messages at the number provided. The following terms apply to our text messaging program:
- Program description: Messages are transactional and account-related, including one-time verification passcodes (2FA), security alerts, and service notifications from AXiS NeuroWellness / Neurological Associates of Long Island.
- Message frequency: Message frequency varies and depends on your activity — for example, you will receive a message each time you request a login verification code. We do not send recurring marketing text messages.
- Message and data rates: Message and data rates may apply. Rates are determined by your mobile carrier and are your responsibility. Please contact your wireless provider for details about your plan.
- Opt-out: You can cancel the SMS service at any time by replying STOP to any message. After you send STOP, we will send a confirmation message and then stop sending SMS messages to your number. Note that opting out of text messages may prevent you from receiving one-time passcodes needed to log in to the patient portal; in that case you may need to use an alternative verification method or contact our office.

- Supported carriers: Carriers are not liable for delayed or undelivered messages. Message delivery is subject to effective transmission by your wireless carrier and is not guaranteed.
- Privacy: No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Information sharing to subcontractors in support services, such as customer service, is permitted. All other use of your information is governed by our Privacy Policy.
